we do not. We used to, but the feedback was not necessary helpful. Teachers were able to use the feedback as they select their objectives for the following year but were not required to share results with their evaluator. This was only done in secondary schools.

We do not ask for student feedback for each class. However, we do include students (I believe from 4th grade up) for feedback in our Culture and Climate Survey that is done every 3 years.
